How to Set Up and Embed a Survey on Your Website

Learn how to create and embed a survey on your website as a popup or button using Lucidya, including display customization, frequency settings, and domain verification.

Lucidya's Embedded Surveys feature lets you collect feedback from website visitors by embedding a survey directly on your site as a popup or interactive button. You can customize the appearance to match your website's design and control when and how often the survey appears.

Step 1. Create a New Embedded Survey

Go to Surveys from the left sidebar and click New Survey.

Select Embed on Website from the options.

Enter an internal survey name, this is for internal use only and will not be visible to participants.

Click Create.

Step 2. Understand the Embedded Survey Builder

The embedded survey builder has three main sections.

Content on the left: where you add question types and create sections to group related questions together.

Builder in the center: where you manage each question by adding the question title, description, and answer choices. Conditional paths are also created here.

Display and Question Settings on the right: the Question Settings tab lets you configure the selected question. The Display Settings tab controls how the survey appears on your website.

Step 3. Add Questions

Click the + button on the Content tab or the + icon within the builder to add a question. All questions require a title. A description is optional. Dropdown List and Multiple Choice questions also require answer options.

Lucidya supports the following question types: Contact Information (Name, Email, Phone Number, and Social Card), Yes or No, Multiple Choice, Dropdown List, Number, Free Text Response, Net Promoter Score, Customer Satisfaction Score, and Rating Scale.

Note that Customer Satisfaction Score is calculated by dividing the sum of positive ratings by the total number of responses and multiplying by 100.

Step 4. Configure Each Question

Use the Question Settings tab on the right to configure the selected question. The following options are available depending on question type.

All question types: mark as required to prevent skipping.

Phone Number questions: select a country code.

NPS and CSAT questions: add colored themes.

Multiple Choice questions: allow multiple selections, randomize options, or add an Other option.

Dropdown List questions: randomize options or sort alphabetically.

Number questions: set minimum and maximum values.

Free Text questions: set a maximum character limit.

Rating Scale questions: set minimum and maximum values and add optional low and high value labels.

Step 5. Group Questions into Sections (Optional)

Click New Section at the bottom left of the Content tab to add a section. Sections group related questions together for a better participant experience.

For each section you can duplicate it to copy all questions inside, change the text direction between RTL for Arabic and LTR for English, or delete it to remove all questions in that section.

Step 6. Customize the Survey Display

Open the Display Settings tab on the right to configure how the survey appears on your website.

Embed Options: choose between Popup or Button format. Set the size as small, medium, or large and choose from up to nine position options on the page.

Button Style: appears when Button embed type is selected. Customize the button title, font color, font type, background color, and corner radius as rounded or square.

Popup Style: customize the alignment as LTR or RTL, pick a primary color, and set the corner radius as rounded or square.

Font Style: select a font from commonly supported Arabic and English fonts and choose a content color.

Backdrop: pick a background color and set the opacity percentage.

Progress Bar: enable or disable to show participants their section progress as they complete the survey.

Hide Sections Header: choose whether section titles are visible to participants.

Opening Settings: choose the animation effect when the survey appears: fade in up, fade in down, slide in left, slide in right, or none.

Frequency Settings: set how often the survey appears: every session for returning visitors or once for first-time visitors only. Set when to stop displaying: after survey submission or after closing the popup.

Step 7. Publish the Embedded Survey

Click Publish in the top right corner. A guided flow will walk you through three steps.

Select Display Scope: choose All Website Pages to show the survey across your entire site, or select Specific Pages and add the URL slugs of the pages where the survey should appear.

Add the Snippet Code: copy the embed code and paste it just before the closing body tag of your website template file. Share this with your developer if needed.

Verify the Domain: enter your domain and click Verify. Once successful the status will change to Verified. If verification fails review the steps and retry.

Once verified your embedded survey will go live on your website.
